CLEVELAND GOLF LAUNCH SQUAD ENROLLMENT FORM
The applicant desires to join the Roger Cleveland Golf Company, Inc. ("Cleveland Golf") Launch Squad. The Launch Squad is an important part of Cleveland Golf's research and development program and its ongoing effort to provide the highest-quality state-of-the-art golf equipment. Members of the Cleveland Golf Launch Squad may be provided with opportunities to test and evaluate certain prototype equipment ("Prototype Equipment"). The testing and evaluation is intended to enable players of varying abilities to test and evaluate the Prototype Equipment. It is the hope and the intention that the participant will enjoy the experience while providing valuable feedback to Cleveland Golf. However, the value of Cleveland Golf’s research and development efforts can be jeopardized or severely compromised if confidentiality is not strictly maintained. Therefore, it is essential that each prospective member of the Launch Squad agrees to the following terms and conditions:
1) |
The applicant represents and warrants to Cleveland Golf that neither he/she, nor any member of his/her family is employed by or is affiliated with any other golf equipment manufacturer. |
2) |
Cleveland Golf reserves the right, in its sole and absolute discretion, to select those participants who will be involved in any testing or evaluation. Participants will not be paid for their participation. |
3) |
Until the Prototype Equipment has been released generally to the public for sale, the participant agrees to keep strictly confidential and not to disclose in any manner (e.g., orally, in writing, or via the Internet) any of the following: |

a) |
The look, feel, appearance, dimensions, specifications, characteristics, color, capabilities and all other features of the Prototype Equipment; |
|
b) |
Any information, whether written or oral, provided by the representative of Cleveland Golf to the participant related to the Prototype Equipment; |

c) |
The evaluation, comments, opinions and statements of the undersigned and any other participants with respect to the Prototype Equipment. |
4) |
The applicant hereby consents to the use, publication, republication and broadcast by Cleveland Golf of any of his/her reactions, statements and comments with respect to, and evaluation of, the Prototype Equipment. |
